RESUMEN

Vesicouterine fistula is a rare condition. Its incidence, however, has been increasing due to the higher incidence of cesarean sections. The presence of a live fetus inside the bladder who passed through a vesicouterine fistula is an extremely rare situation. We report a case of woman who underwent two previous cesarean sections, was referred to a hospital due to mild pelvic pain and genital bleeding. At the moment, physical examination was normal. Ultrasound scan revealed a gestational sac inserted into the anterior wall of the uterus, with a living fetus of approximately 13 weeks, with active body movement and normal heart rate inside it. The fetal abdomen, around the waist, was stuck at the opening of a vesicouterine fistula, so that the fetal head and trunk were entirely into the bladder cavity, while lower limbs remained at the uterine cavity. Laparotomy was performed, the fistulous tract was excised, the fetus (without heart beating) was removed on opening the bladder, and the uterine cavity was emptied. The defects in the bladder and uterus were repaired. The postoperative period was uneventful. A live fetus inside the urinary bladder is a rare condition the continuation of pregnancy is unlikely and the vesicouterine correction can be made by the time of surgical intervention.

RESUMO A fístula vesicouterina é uma condição rara. Sua incidência, no entanto, vem aumentando, devido à maior incidência de cesáreas. A presença de feto vivo dentro da bexiga por meio de uma fístula vesicouterina constutui situação extremamente rara. Relatamos o caso de uma mulher com duas cesarianas anteriores encaminhada para o hospital devido à dor pélvica leve e sangramento genital. Na hospitalização, o exame físico estava normal. A ultrassonografia revelou saco gestacional inserido na parede anterior do útero com feto vivo de aproximadamente 13 semanas, com movimento corporal ativo e frequência cardíaca normal. O abdômen fetal, ao redor da cintura, estava preso na abertura de uma fístula vesicouterina de modo que a cabeça e o tronco fetais estavam totalmente dentro da cavidade da bexiga, enquanto os membros inferiores permaneciam na cavidade uterina. A laparotomia foi realizada, o trajeto fistuloso foi excisado, o feto (que estava sem batimento cardíaco) foi removido ao abrir a bexiga, sendo a cavidade uterina esvaziada. Além disso, foram reparados os defeitos na bexiga e no útero. O pós-operatório transcorreu sem intercorrências. Feto vivo dentro da bexiga é uma condição rara, e a continuidade da gravidez é improvável, sendo que a correção vesicouterina pode ser feita no momento da intervenção cirúrgica.

CONTEXT: Renal artery pseudoaneurysm is a rare complication after renal injury but should be suspected whenever there is recurrent hematuria after renal trauma. CASE REPORTS: We present three cases of pseudoaneurysm after blunt renal trauma and a review of the literature. All patients underwent renal angiography. Two cases were diagnosed during the initial hospital stay due to hematuria, or in the follow-up period during recovery. One patient was hemodynamically unstable. Two patients successfully underwent coil embolization in a single session. In the other case, selective embolization was attempted, but was unsuccessful because artery catheterization was impossible. Procedural and medical success and complications were retrospectively assessed from the patients' records. The clinical presentation, treatment options and clinical decisions are discussed. CONCLUSIONS: Renal artery pseudoaneurysm may develop acutely or even years after the initial injury. Signs and symptoms may have a wide spectrum of presentation. Selective angiographic embolization is an effective treatment that reduces the extent of parenchymal infarction.

CONTEXTO: Pseudoaneurisma de artéria renal é uma complicação rara após lesão renal, mas deve ser suspeitada quando houver hematúria recorrente após trauma renal. RELATO DE CASOS: Três casos de pseudoaneurisma após trauma renal fechado são apresentados, além de revisão da literatura. Todos os pacientes foram submetidos a angiografia renal. Dois dos casos foram diagnosticados durante a estadia hospitalar inicial, por hematúria ou no período de acompanhamento durante a recuperação. Um paciente estava hemodinamicamente instável. Dois pacientes foram submetidos a embolização com coil com sucesso em uma única sessão. No outro caso, a embolização seletiva foi tentada sem sucesso, devido à impossibilidade de cateterização da artéria. O sucesso médico e do procedimento e as complicações foram retrospectivamente avaliados a partir dos registros dos pacientes. A apresentação clínica, opções de tratamento e decisões clínicas são discutidas. CONCLUSÕES: Pseudoaneurisma de artéria renal pode se desenvolver agudamente ou até mesmo anos após o trauma inicial. Sinais e sintomas podem ter um amplo espectro de apresentação. Embolização angiográfica seletiva é um tratamento efetivo, reduzindo a extensão de infarto parenquimatoso. .

CONTEXT: Ureteral fibroepithelial polyps are rare benign nonepithelial tumors, and less than 200 cases have been reported in the literature. We report on a pregnant patient with ureteral fibroepithelial polyps that were successfully treated with laparotomy. CASE REPORT: A 23-year-old pregnant woman presented with a three-month history of intermittent lumbar pain of low intensity. Abdominal ultrasonography showed that she was 13 weeks pregnant and found severe left-side ureterohydronephrosis and a heterogeneous solid mass measuring 11 x 8 x 7 centimeters in the middle portion of the ureteral topography. The investigation was complemented with magnetic resonance imaging, which confirmed the previous findings. Nephroureterectomy was performed without complications. The specimen revealed three solid tumors in the ureter, of which the largest was around eight centimeters in length. The anatomopathological report confirmed that they were fibroepithelial tumors without malignant components.

Nós reportamos o caso de uma mulher grávida com pólipos fibroelitelial ureteral que foram tratados com sucesso por laparotomia. RELATO DE CASO: Mulher de 23 anos de idade, grávida, apresentava história de dor lombar intermitente de baixa intensidade há três meses. Ultra-sonografia abdominal revelou gravidez de 13 semanas, ureterohidronefrose grave à esquerda e massa sólida heterogênea medindo 11 x 8 x 7 cm na porção média da topografia ureteral. A investigação foi complementada com ressonância magnética, que confirmou os achados. Foi realizada nefroureterectomia sem complicações. A peça revelou três tumores sólidos em ureter, o maior apresentando aproximadamente oito centímetros longitudinalmente. O relatório anatomopatológico confirmou tumor fibroepitelial sem componentes de malignidade.
